️🇺🇸WaPo: A US military investigation has found that multiple system failures contributed to the mid-air collision between a Black Hawk helicopter and an American Airlines passenger jet last year near Washington, D.C., that killed 67 people.
The military failed to address known accuracy problems with altimeters on older Black Hawk helicopters, adequately inform pilots of the risks, or systematically collect data on near misses and train crews for the dangers of operating in Washington's congested airspace.
The report found that the helicopter's altimeter read 200 feet when the plane was actually flying 278 feet above the Potomac River.
The Army has since issued a service-wide notice about potential altimeter errors, but a broader update to operator manuals is not expected until 2027.
