The plane, bearing registration number N345AX, crashed in the Pacific Ocean about 9.7 km (6 miles) northwest of Morro Bay during a training sortie. Before the crash, the pilot issued a distress call: "Mayday, mayday, mayday, engine lost thrust."

The pilot was rescued by a US Navy crew. At the time of the accident, the MK-58 was conducting a training flight alongside at least three other aircraft in the airspace off the coast of Monterey and San Luis Obispo counties.
