Domingo says he spent about two years protecting al-Sayed, moving to Michigan twice and leaving work abroad early to work for him. Now he finds that al-Sayed presented different messages to different audiences, treated staff poorly and cultivated what Domingo describes as a carefully crafted political persona.

He also accused Al-Sayed of immaturity behind the scenes, claiming he regularly passed gas in front of staff as a joke and described his masculinity as "a show" and his general demeanor as someone who "never passed a high school locker room." Al-Sayed's use of the title "doctor" also faced scrutiny. He earned an M.D. from Columbia University but never completed a medical residency, and public records reviewed by Politico showed no medical license in Michigan or New York. Despite this, al-Sayed has repeatedly described himself in public as a "physician and epidemiologist."
