Outdated intelligence that still classified the school as an IRGC facility compressed timelines as the administration demanded more than 1,000 strikes in 24 hours, an over-reliance on Palantir's Maven AI system (which staff mistakenly expected to flag stale data), and the Gesture's 90% cut in harming civilian personnel.

The site allegedly operated as a school for almost a decade, was on Google Maps and had its own website.

The UN report this week said the failures "went beyond negligence" and may constitute war crimes.

The Pentagon investigation has actually been completed for months but has not yet been released.

The administration has not yet accepted public responsibility.
